You can visit any bank (I recommend NMB bank ) with you citizenship and ask for dollar card, they will ask necessary information and documents and provide you the card in few days, you can add balance to those card by asking bank to do and paying NPR to Bank.
You can make dollar card from any commercial bank where you hold your account. You will be needing a copy of your Personal Account Number(PAN) as for official document.
As for cost there is certain charges for making the card although usually all bank provide free first-time load. The charges for almost all the banks can be found on this link.
Visit your nearest bank most banks now provide it . I think NMB bank provides it free of cost. You need to have PAN number which you can get from Nagarik app.
First of all, you need a bank account that must be authorized by class ‘A’ commercial banks and class ‘B’ development banks.

Nepal Rastra Bank granted the issuance of prepaid dollar cards on 21st March 2021.
You can easily apply for a Dollar card in Nepal from the bank providing this service. You are allowed to take prepaid dollar cards from a single bank only. Applicants need to hold NPR accounts in their respective banks to be eligible for this service. In addition, the following documents are mandatory:

As NRB has not fixed any charge for the dollar card, the charges may vary from bank to bank. In general, the amount loading fee is Rs. 500, the customers have to pay this charge each time they load money into their dollar account. You may need to pay subscription fees, security fees, re-issue fees as per the respective banks.
